Youth Connect Outreach Program is a one-to-one early intervention program designed to help young people (13-19) who are in need of extra support. The goal of this program is to help youth stay or become connected to their family, community and school and to strengthen connections to natural supports in order to prevent and/or reduce the risk of homelessness, isolation and substance misuse.
Families, social workers, teachers and counsellors can submit a referral via email Child, Youth & Family Connections to connect a youth with a one to one Youth Connect worker.
